For your first visit, we will need:

  1. Valid Government-issued ID
  2. Current Insurance Card and information
  3. All current medications in their original pill bottles
  4. Any other physician or radiology reports/CDs that you have
  5. Contact information for your Primary Care Provider
  6. Completed New Patient forms
